Drag and drop the fields you want to use from the menu on the right
You should see first name, last name, email, phone, by default
Click Custom Fields if you want to make a new field for your form
Click the green button "Add custom field" if you want to make your own custom field
Note: if you've already created custom fields, you'll have to scroll all the way to the bottom of your custom fields to find this button
After the field is created, drag and drop it onto your form
When you're finished creating the form, click, "Integrate Form" on the bottom right
Note: You'll notice that a link/embed appears - you can copy this link and include it in your emails, texts, or funnels.
